Dear Sir, 
                                           
                                          Leave of absence

This is to advise you that I could not come to office on Monday, the 11th of December, 2016, as I had to take my mother to the hospital when she suddenly felt unwell after breakfast. She has since been admitted in the hospital under the care of a renowned cardiologist.

I regret my sudden absence, but trust you'll appreciate I was helpless under the prevailing situation at that point of time. 

Kindly pardon me for not attending office and please issue suitable instruction to the Human Resources Department so that they are able to adjust my absence against the casual leave due to me.
