Respected Sir,
I am (name), (Class/Grade) of the (School / Institute name) writing for a leave of three days (As your requirement) due to my mother’s illness. Yesterday my mother suffered a lot after heart-attack (show actual cause) because there was no one to help her while I was in the school/college/university. Returning to home I took my mother to the hospital and still she is there in the hospital.
