Chromatin is a mass of genetic material composed of DNA and proteins which is found in the interphase of the cell cycle. And chromatin condenses to form chromosomes. So chromosomes are just condensed form of chromatin found in the mitotic phase of the cell cycle (It is more organised than chromatin). Each chromosome (unreplicated form) consists of one chromatid. OR by definition Either of the two daughter strands of a replicated chromosome that are joined by a single centromere and separate during cell division.

So all this gives us a relation that -
Chromatid makes chromosomes and chromosomes decondense to form chromatin.
