CP of 1 book = 1
CP of 16 Books = 16
SP of 16 Books = 12
16 - 12 = Rs 4 loss

Define x:
Let the cost of 1 book be x
Cost of 12 books = 12x
Find the selling price of 1 book:
Selling price of 16 books = 12x
Selling price of 1 book = 12x ÷ 16 = 0.75x
Find the loss:
Loss = Cost Price - Selling Price
Loss = x - 0.75x = 0.25x
Loss Percentage = ( loss/cost price ) x 100
Loss percentage = (0.25x / x ) x 100 = 25%
Answer: The loss is 25%
