The formula is :
D² = [ ( x2 - x1)² + (y2 - y1)²]
------------------------------------------
So 5² = ( 1 - 4)² + (0-k)²
Or, 25 = 3² + k²
or, k² = 25 - 9 = 16
So, k = ± 4
