When the lieutenant is injured, he shows the emotion of surprise. Despite knowing the fact that wars cause injuries, he is astonished by his injury. 

This was so because the lieutenant was not hired to perform major war tasks but was there for the basic housekeeping job, and his injury surprised him. People around him were also surprised to see him injured. His shoulder was injured.
