If water is added to a concentrated acid, the heat generated may cause the mixture to splash out and cause burns. The glass container may also break due to excessive local heating.

Strong acids are when added to water it releases more amount of heat makkng more concentrated solution and this reaction is harmful test tube may break while doing this.To avoid this waters are dropped into acids not acids into water.
