If you have any hobbies, I'm sure she would like to hear about that. I think she would also like to hear about what you do with the family. She misses that part of seeing you grow up, and it will be her only way for her to enjoy that part of your life. You may want to ask her questions about her life in Germany, how it was for her growing up, and hobbies that she enjoys. Show an interest in her, and I'm sure the conversations will be easier. You should send pictures of yourself...maybe of clothes that you admire, things that you are interested in. I'm sure anything will be greatly appreciated. Grandma's are a treasure that you realize only when they are gone...so take advantage of the time you have to spend with her, even if it is only in letters.
