ur letter's body iam giving you.
U can write it as:
iam(ur name) studying in (ur class) took leave from (so and so date). I was unable to come to school because i was not feeling well. So please permit me to sit in the class.
If needed u can also attach ur medical receipt with the application.

but he said he want to give this letter to his teacher ad not to principal
