This word is commonly used as adjective like:-

1)  Entirely irrational behaviour i.e. U can say Mad or Insane.
     Can be used in frankly talks with folks and mainly used for mad one! :D
 

2)  Affected by the loss of intellectual functions that is associated with dementia.
     This is mainly used for OLD  people

In HINDI it can be termed as:-
 
Unnmat, Deewana, Pagal, Bawla, Vikshipt....
