The first aid was provided by my father

Explanation:

When changing the voice of a sentence in simple present tense it gets converted into simple past tense. Hence, as we were changing the voice from active voice to passive voice we changed the simple present tense of  'provides' into the simple past tense in the form of 'provided'. Also, another rule is that the subject becomes the object and vice versa. As we see here, 'father' was originally the subject and in our answer 'father' becomes the object and 'first aid' becomes the subject.
