It is true that only the bearer knows wherethe shoe pinches, it simply means that a person can only feel the pain when he suffers from it. The postmaster was not able to understand Ali's feelings and his pain and therefore insults him. But when his own daughter falls ill in another town, he realises what Ali must have felt for long 5 years.

According to the story, The Letter, Post office employees used to read Muhammad Ali's letters to mirrium and make fun of those letters.
