i have a lot of guesses! try all of them:-
1) depression.
depression is also a bit associated with mental pain! but it is really dangerous!

2)headache.
it can also be headache; most people have headaches when they are worried or anxious!

3)indigestion and vomiting.
it is associated with stomach. it is scientifically proved that stomach gets affected when we have hyper tension or stress. 

4)Shivering and sudden fever.
students used to have fever when exams are arriving. this fever has a nick name that is 'fever due to exam fear' or 'exam fever'. it is due to over anxiety.
 
5)body pain
our brain gets functioned up when we are tensed; our heart beats increase and thus it's commonly seen associated with body pain and head aches!
