Hy,
Physician is generally a MBBS doctor, who treats all the diseases. Who gives medicines, vaccinations, health advises. Basically, Physician is your family doctor.
Cardiologist is a doctor who has done his Postgraduation in cardiology, i.e.,he's the specialist of heart. He treats diseases like heart attacks, cardiac arrest, angina, rheumatic fever, hypertension, hypotension, etc.
Diabetelogist is a doctor who is an expert in treating diabetes. They can also be called as endocrinologist.

Diabetologists refer to those physicians who treat only diabetic patients, whereas, endocrinologists specialize in endocrinology and treat all diseases that impact on the endocrine organs. Since diabetes is one of the problems of endocrinology, so endocrinologists can also offer treatment to diabetic patients.
