I can't write a full answer but I'd like to suggest you some tips...
1. Look at the Diary writing format and try to write it such.
2. Then, if you can talk with your parents about your fears, try to talk to them..
3. Write down the measures you can take against your fears separately.. You can ask your parents or siblings about it if you don't know how to..
