It's you who is to blame.

Firstly, in case you might be wondering why I used IS when YOU is are, it's cuz of the pronoun "who" which is singular and so is the verb IS, in agreement to it

Secondly, WE SAY "TO BLAME", like I'm to blame, he's to blame...We don't use the BE form of verb in passive with blame, in fact this is not in passive, it's in active voice, and be form is only used in passive.

So, this is a case of an active infinitive, TO BLAME, not to be blamed, since to blame is an idiomatic expression.
