affirmative sentence - subject + has/have + V3 + object
negative sentence - subject + has/have + not + V3 + object
interrogative sentence - has/have + subject + object + ?

Present Perfect Tense rules

Positive (affirmative) sentence
  Rule: Subject + has / have + 3rd form of verb + object
  Example: I have done my homework. 

Negative sentence
  Rule: Subject + has / have + not + 3rd form of verb + object
  Example: I have not done my homework. 

Interrogative sentence
  Rules: 1) Has / Have + subject + 3rd form of verb + object + ...? 
              2) Question word (What, why, etc.) + has / have + subject + 3rd form of verb + object + ...? 
                You have to add a question mark in the end. 
  Examples: 1) Have you done your homework? 
                     2) What have you done in this year? 

Here, '3rd form of the verb' refers to the 'past participle form of the verb'.
