True, iron once rusted, cannot be brought back to it's original form because an oxide layer is formed on it which slowly eats the metal away due to corrosion.

yes it is an irreversible change as rusting of iron makes ferrous oxide

we can only obtain it by laboratory process which removes the oxygen by electro-chemical ways
