Hey

 \sqrt{427 + \sqrt{181 + \sqrt{?} } } = 21 \\ \\ squaring \: on \: both \: sides \\ \\ = > 427 + \sqrt{181 + \sqrt{?} } = 441 \\ \\ = > \sqrt{181 + \sqrt{?} } = 14 \\ \\ again \: squaring \: on \: both \: sides \\ \\ = > 181 + \sqrt{?} = 196 \\ \\ = > \sqrt{?} = 15 \\ \\ = > ? = 15 {}^{2} = 225
