I usually hear people say San Franciscan or similar when referring to the SFBA and it's clear that the conversation is not about finer geographic issues. Just like if you're from Cambridge, MA or Newton, MA, you're a Bostonian so long as you aren't speaking about more issues. People understand that you're speaking about the San Francisco Metropolitan Area (i.e. the SMSA) rather the the city itself
