The best way to do it is to apply the two colours side by side closely and they take a wet brush without any colour and move it back and forth on the line where the two colours meet until you are satisfied with the blending.

Also, if you are doing two colours one light one dark such as yellow and red, then use a little yellow on the brush to blend.
