The phrase tries to present the bonding between-

 \bf {Blood\: relation\:and\:relation\:with\:outsiders.}

Blood relation is always stronger than mere friendship formed through acquaintance and it is more valuable than anything else.

Family gives us everything like food, shelter, and everything. Blood relations are lasting. They have a more strong bond of affection than all other relationships.

Sometimes we may share very strong bond with our friends than with our family. And it is also possible that we may have fights or sometimes jealousy feeling with our brother sisters but that doesn't ends our relation. But once if the jealousy and fight takes place in other outside relations it lasts (ends up). Sometimes we may find sincere friends who is willing to help but they often stop when they find themselves to be in troubled waters. But blood relatives such as father, mother will stand by our side risking even their lives.

True and sincere friends are often hard to get and even if we find one who is true and sincere, we cannot expect the greatest sacrifice from them. They give more importance to their own family. So even if true and sincere friendship is to be given due credit, we should value blood ties more. They last longer and are often found to be stronger than the latter.
