The nitrite then reacts with oxyhemoglobin (the oxygen-carrying blood protein) to form metheglobin, which cannot carry oxygen.

Blue baby syndrome

Blue baby syndrome is a fatal condition.

It occurs when the hemoglobin (Fe2+) in an infant's red blood cells is oxidized to methemoglobin (Fe3+). Methemoglobin is unable to transport oxygen.

Affected infants develop an unusual blue-gray or lavender skin color.

Methemoglobin levels > 50% can quickly lead to coma and death if the condition is not recognized and treated.

The most common environmental agent associated with this condition is nitrate-contaminated water.

Infants are particularly susceptible to the condition during their first 6 months of life because they have low amounts of methemoglobin reductase, a red blood cell enzyme that converts methemoglobin back to hemoglobin.
