It signifies the temp at which the change in state occurs. for example , here, ice cubes(solid) starts melting at 0° C to form liquid. then, that liquid starts getting converted into vapour at 100° C . Again when condensed, the vapour started condensing back into liquid when temp starts falling below 100° C . further when the temp falls below 0° C , the water starts converting back into solid.
