The phrase is a kind of meaning for the situation. Brutes was caeser's best friend. To be stabbed by him was most painful as he would have never thought of being hurted by him. if it would have been someone he would have been rather comfortable. that's why it's said as " most unkindest cut" or most unfavourable situation for brutes when his friend is going to stab him.
