A symptom is any subjective evidence of disease, while a sign is any objective evidence of disease. Therefore, a symptom is a phenomenon that is experienced by the individual affected by the disease, while a sign is a phenomenon that can be detected by someone other than the individual affected by the disease

A symptom is a departure from normal function or feeling which is noticed by a patient, reflecting the presence of an unusual state, or of a disease. A symptom is subjective, observed by the patient, and cannot be measured directly, whereas a sign is objectively observable by others.

A disease is a particular abnormal condition that negatively affects the structure or function of part or all of an organism, and that is not due to any external injury. Diseases are often interpreted as medical conditions that are associated with specific symptoms and signs.

To make it clear lets take an example:

Joint pain in the knee is the symptom of the disease osteoarthritis.

If you feel that you are having pain in the joints like knee,hip,elbow etc., this may be the symptom that you may have osteoarthritis.
