When we increase the temperature and pressure on a state of matter the kinetic energy of the particles of matter increases. As the kinetic energy increases, the particles become excited and start moving more freely. A stage comes when these particles leave their positions and change of state takes place. Similarly when we decrease the temperature the current state of matter loses kinetic energy and gets converted to its previous state.
