Wormhole, as I've said earlier in the comments are a link between two specific points in space.
It is like a tunnel that connects two different points, but is relatively smaller than the actual distance between those two points.
But wormholes are hypothetical, that is their existence is not proven, they are just observed in theory. They have just being visualized as a solution to Einstein field equations.
