In English, there is no active/passive difference for “is”. So there is no answer to your question.

“What is the name of your father?”
The passive voice is used when the subject is acted upon: "She was hit in the eye by the snowball."
Instead of the active voice: "The snowball hit her in the eye."
The object of the active sentence becomes the subject of the passive content.
