Well.. mostly it depends on passion but keep in mind it can up to 12 yrs to become a Great doctor(my sister is a doctor herself), I'm an aspiring Physicist so I would ask you to give science a fair chance too, try it you will love it.
Follow whichever one your gut says to follow exploit it to the best you will earn a lot, scope wise I would say commerce has extremely wide scope your opportunities widen exponentially whereas currently medical leads to one way doctor or nurse etc.
Biotechnology is developing but it has not fully grown plus jobs in biotech don't pay well
