We have inserted the helping verb 'is' because, it is given "(be)" in the question which means to insert a be-form verb there. And why we have inserted "is" not am or are is because the noun in the given sentence is in singular form.

Now coming to the second blank, here it is given "not help". So we have to fill it with correct form of verb. The correct form of verb which should be used here is Present Continuous Tense. And present continuous tense forms in the structure- Subject + am/is/are + verb(-ing) + ... So we have added the word "help" with the suffix (-ing) and got the new word "helping". At last we got a meaningful sentence - "Why is he not helping you?".
