Saline is administered instead of distilled water because water is hypotonic relative to the contents of the blood and it may cause the cells to burst, whereas saline due the presence of salts is hypertonic relative to the contents of blood.

Doctors administer saline to maintain isotonicity with body fluids

Explanation:

Saline has many uses in medical field. For example, it is administered intravenously to replenish water and salts in patient's body. Saline has usually 0.9% sodium chloride which is the same as the concentration of salt in body fluids. Hence, both of them are isotonic to each other.

Distilled water has no ions hence it is hypotonic compared to body fluids. If it is administered in place of saline, water will move from high to low concentration which means it will move inside the cells. The cells might swell and also burst. Hence, it is not administered to patients.
