There are many reasons for people becoming lean and thin. A few include:
1. Malnourishment
2. Vigorous physical activity in excess
3. Malabsorption syndromes
4. Pathological conditions like Cancer, Tuberculosis etc.
5. High content of brown fat

Some people are very thin because they have the genetics which give them a fast metabolism and a resistance to store extra energy as fat.
And, guess what? They are "normal looking"- for them, anyway.
Thin people have another thing going for them, they continue to be able to maintain their weight much more easily as they age- very helpful because, as you age, your metabolism slows down.
If a person is so thin they are nutrient deficient, then they should see a doctor to find out what is wrong- could be a thyroid problem, carcinoma, intestinal creature or inability to absorb certain nutrients- but the thing is, they would also be sick- they'd appear and feel not just thin, but also unwell; so, if a person is very thin and not sick, they probably are just lucky in their genetics, and should simply live their life without worrying about looking like every one else.
