               Sub: Request regarding change of date of birth in aadhaar card.

                I am studying in 9th class B section of your school. During issue of aadhaar card my date of birth wrongly printed as 15.08.2000 instead 15.08.2002. For change of date of birth in aadhaar, its required to submit a declaration from school.

                In this connection, it is requested to issue a date of birth declaration from our school for submitting to aadhaar issuing agency please.
