sir/madam,

I the student of your school have the honour to state that I am feeling ill now ,my doctor who is treatment me advice me to rest at home for at least five days till not my recovery. so I can't come to school and attend the classes from 1stJune to 5th June.

Hence I therefore kindly request you to sanction medical leave to me for the above reason for which I shall remain ever grateful to you
