Urine formation occurs in the kidney in each nephron. The kidney function involves:-

Ultra-Filtration:- ultra-filtration occurs in the glomerulus.
It is the filtration under high pressure.
This occurs because at the arterial end of glomerulus, there is a high blood pressure which forces out the liquid part of the blood, including dissolved substances like salts, glucose, urea and uric acid.
Plasma proteins and blood cells are too large to pass through.

The filtrates from the glomerulus passes into the renal capsule and move into the renal tubules.

SELECTIVE REABSORPTION:-
It occurs in other parts of the nephron that is
In the first convulated tubule, where, glucose, some water and most of the salts are reabsorbed.
In the Loop of Henle, the remaining water is reabsorbed.

IN THE SECOND CONVULATED TUBULE:-
More salts and water are reabsorbed.

Urea,Uric Acid and Salts which are not needed by the body together with excessive water, continue to move down the collecting duct into the Pelvis of the kidney,
Down the Ureter to the Urinary Bladder.
This solution in the bladder is known as Urine, which is expelled out of the body by the contraction of urinary bladder and the process is called as urination or micturition.
Hope it helps.
