Dear sir,

I am Vidhi/ Vishal, the citizen of (your area). I wanted to inform you that the road in our area is in very bad condition. There are holes in between the roads and they are frequently one after one, it creates a lot of problem in driving and sometimes lead to small accidents. The roads are broken from the sides. Also, there are speed bumps on the road at a very high height which is also a problem. The bumps should be small and at a equal and little long distance but it is opposite in my area road.

with a humble request I want you to check on this matter and fix it as soon as possible.
