Sub: Mistakes in Adhar card.

I'm writing this letter to inform you that I recieved my adhar card yesterday. While going through few vital information , have got some mistakes which are very necessary to be corrected.

The major mistake was miss spelled name. As my name is Anuj Tripathi , in Adhar card it has been printed as Ankuj Tripathi. I don't know how this happened , while I provided my every information in printed form. Second mistake was name of mother and father exchanged. In place of mother's name it has been written Rudra Tripathi. And on father's name they have printed it as Mantra Tripathi. Seeing such mistakes was shocking for me. After that I realised that even the address was wrong. Its a very urgent need for me to arrange my Adyar card as soon as possible. I cannot use it anywhere with these wrong information.

I'm attaching few of my credentials with clear declaration. Please do correct it or else I won't be able to sit in exam. please sir, it's my request to make it fast as of 12th October. It's a very important exam. I would be grateful to you if you make it fast anyway.
