Axiom is basically the basic facts which are accepted universally without any proof.

\large\underline{Theorem}

Theorem, however, is the inference which is drawn through particular/different logical reasoning using proven results (mainly the previous ones) and/or some of the axioms.

The basic difference is just that theorems comes with sufficient proof and evidence whereas axioms don't require certain evidences.
