Heat is a form of energy that travels a hotter body to a colder body. As a water bag contains more heat energy compares to the injured back heat travels from the water bag to the injured back.(mainly through conduction and convection)

in comparison to water present in our body the hot water in water bags are less saline and water moves from less saline content to high content so when the water in bags which are less saline moves to our body water which are more saline.
