How to write a letter to bank about I
have lost my credit card while traveling from Mumbai to pune .write a letter to the bank requesting for a new credit card.

Dear Sir/Madam,
My name is John Peter. I have been a regular member of sky bank, with account number 4400889922. I am writing to inform you that I lost my credit card on the 9/01/17.
The last time I recall having my credit card with me was in Coles super market, when I went shopping for my family. Though, I remember putting my wallet on my shopping trolley. But, when it was time to make payment for the things I pick from the store, I could not find my wallet that contain all my cards.
Sir, I would like my lost credit cards to be cancel to prevent it from been used by thieves. And I would like to my lost credit card to be replace with a new cards to enable me pay my bills and do some shopping.
I will appreciate your timely intervention on the issue. I am looking forward to your favorable response in getting me a new card.
