I especially would like to know what British usage is on this. Perhaps the usage is changing? I've got a British author who keeps writing "by cash" and it didn't sound wrong to me until someone questioned it.

I believe that we can use all of the following:

1) He paid cash.

2) He paid in cash.

3) He paid by cash

4) He paid with cash.

Sentences 1) and 2) are the by far the most frequent.

Sentences 3) and 4) are not frequent, but they are not incorrect, either.

I think in the past, students were being tested on the preposition to use before "cash" and "credit card", even though it might not have actually been in the syllabus. Any little detail in the book would be picked up by the test writers.
