The medal is shaped as a 1.375-inch-diameter bronze disc that is round in form (3.49 cm). On a raised circle, the national anthem is located in the centre.

Around it are four imitations of Indra's Vajra. A straight swivelling suspension bar is used to suspend the medal. On edge is its name. The Param Vir Chakra is suspended on a 32-millimetre (1.3 in) long purple ribbon. Two legends are divided by lotus flowers on the back, surrounding a simple centre. Hindi and English are used to write the phrase "Param Vir Chakra."
