Heavy water is just the similar water but this time the two atoms joined in a covalent bond with the oxygen atom are going to be the isotopes of hydrogen having two neutrons that is deuterium (often denoted as D)

This is represented as D2O.

D2O is generally prepared by hard electrolysis of H2O.

And heavy water is referred to as it is because this time we have different molecular mass of this molecule due to the presence of heavier atoms of Hydrogen having individual atomic masses 2 instead of 1 due to the presence of that extra neutron in each of their individual nuclei.

Just like how when Water reacts with Na metal and forms NaOH. Again similarly here, NaOD will be formed.
