A bank locker is a 'Safe' or a 'Vault' or a 'Locker, that is rented out by banks in exchange for a certain fee. The lockers are guarded heavily and made out of strong metals which can't be easily forged into. These lockers can be used to store physical valuables like jewellery,legal documents and agreements,cash etc.The bank protects the locker on our behalf and we pay locker rent to the bank for that. Banks are supposed to rent out the lockers on first come first serve basis. When a bank sets up a safety vault in one of its branches, it is supposed to take applications from its customers who are interested in taking lockers. we can say that the application form which I have mentioned earlier,is just the same as a locker requisition form. There are various types of requisition form available in banks for deposits/articles left in locker, for example (1) Form for nomination on deposits/articles in locker: (2) cancellation of nomination for deposits/articles in locker: (3) Change in nomination for deposits/articles in locker etc. In my knowledge There is no such locker requisition form available in any bank. Either you have to write an application, if you need a locker or you can mention your request of locker in a comprehensive prescribed form. I did exactly this when I applied for ATM, Cheque Book and Locker in Oriental Bank of Commerce. I just ticked on the facilities mentioned in the form, that I wanted from the bank.
