causative microorganism refers to the organism which is actually causing the disease for example plasmodium causes malaria and Salmonella causes typhoid but if we mention that mosquito causes Malaria then we are wrong because it is the vector of the disease not the causative organism

The word causative agent refers to harmful and disease causing substance so causative microorganisms are pathogenic, disease causing microorganism.

Most common examples of pathogens that cause disease in human beings are Salmonella, Listeria, E coli. These are bacterial pathogens.

An example for viral and protozoan pathogens is cryptosporidium and plasmodium respectively.
