The answer is number 4, because you never want any medications to interact because it could be detrimental to the patient.

It isn’t numbers 1-3 because a Doctor would decide the treatment, a therapist would help emotionally/mentally support them, and they don’t have to communicate the decided treatment because that wouldn’t be in their job description.
