Formula of calcium hydroxide is Ca(OH)2.

It is made up of one calcium atom, two hydrogen atoms and two oxygen atom. The oxygen and hydrogen atom together form the hydroxide ion and it gets attached to calcium ion to form calcium hydroxide.
