You stomach is an acid-filled bag. To stop the acid from corroding the bag itself, the inner lining secretes mucous, gross. When you eat there's a complex systems of hormones that stimulates the secretion of stomach acid, along with the protective mucous membrane. Some medicines, particularly non-steroidal anti-inflammatories (aspirin, ibuprofen), inhibit the secretion of mucous by the stomach lining, which can really cause problems as explained above. This is why you have to eat BEFORE, in other words, you ensure that there's already a protective mucous layer BEFORE you take the drug. This is the first reason, and a good one, if you don't want to digest yourself.
