this could be due to the following reasons
1) cities are mostly polluted and when children play in muddy water and sand it's usually unhealthy and as you know they do not tend to clean their hands before food so these germs can travel into the body easily.
2) most of the children may not be regularly vaccinated in cities as maybe their parents are busy with theirs jobs that they don't find the time for regular health checkups
3) unhealthy eating habits such as eating in food stalls outside such as pani puri etc may not be prepared from good oil and as you know disease transmitters such as flies and insects may sit on the food that they consume.
4) common ways of spread through air, water and food in offices, public areas such as parks beaches, schools etc
5) even sometimes hotels may not clean their bed sheets often which can cause a more quicker way to spread.
6) plates people eat in hotels should be sterilized in pure hot water when not clear properly disease are very highly prone to spread
hope it helps you!
